This report form is a guide. Feel free to make copies or add additional information on the back of the page to fit your shoot. If a cashier’s trophy report or your own shoot report is easier for you to use, that is fine too.

Please note weather conditions, first time achievements, special shoot events or special trophies. Sending a copy of the shoot program is helpful. Photos add to the story. Please write names of shooters on the back. We will return your photos if requested. Digital photos are also fine, if sent on a disc.
